Technology Work Request

Technology Work Requests (TWRs) should be submitted from an AS400 display session (Press Esc, then F2, then 4) by, the Site Contact Person for your campus. A TWR should contain:

  1. Site Code:

      The is the building code for your site. If you don't know your site code check the NPS phone list. Next to each school name is the corresponding site code. You can also check with your site secretary or building administrator.

  2. Requestor:

      The name of the user who is logged into the AS400 automatically appears in this field.

  3. Specific Location:

      The room number or area where the troublesome equipment is located. Please also include the name of the person who uses this equipment and who may be able to provide more details about the problem.

  4. Request:

    • Equipment: Please state the manufacturer and model or type of equipment with the problem (e.g., "Dell GX270" or "Latitude D505"). Listing the NPS FAS# may help us to determine which particular piece of equipment has the problem. The FAS# is usually a 6-digit number on a white or metallic sticker attached to the equipment. The stickter is usually only applied to the CPU, monitors, keyboards, mice and printers do not have FAS#'s.

    • Software: If you suspect the problem may be with your software (computer programs or operating system) then please also provide us with the name of the software package(s) in which you notice a problem as well as the version of the operating system you are using (e.g., "Office 2003 on UDT" or "Internet Explorer on Windows XP").

    • Nature of Problem: Give us some idea of what the problem is, when it occurs, and what you have attempted to solve the problem.

    • Details: "The devil is often in the details" and the more relevant details we have, the sooner and better we can exorcise your problem.

Please list only one problem with one piece of equipment per TWR.
