Enrollment Requirements Medication	MADISON ELEMENTARY SCHOOL Important Forms Enrollment Requirements All children age five (5) on or before September 1 are eligible to enroll in kindergarten. All children age four (4) on or before September 1 are eligible to enroll in PreK. Important Information At the time of enrollment the following information is needed: Official birth certificate Social Security Card for child Immunization Record (see below) Two proofs of residency!
